Transition House’s rapid rehousing program helps homeless families with children move back into permanent rental housing while providing ongoing case management aimed at keeping those families in stable in housing.
Transition House has provided cash rental assistance for homeless families or households in grave danger of becoming homeless since 2009. Transition House’s program offers accompanying case management for families receiving assistance for a minimum of six months and a maximum of 24 months. Case management offers a holistic, anti-poverty approach designed to address the root cause of poverty, help families gain employment skills, increase income, and develop household money management techniques. Case management also offers family members assistance accessing entitlements, medical or psychological counseling services, and other community support. Finally, the program employs a housing locator that helps families find affordable housing, and on average 70 percent of our sheltered families successfully return to permanent rental housing.
